Do you offer telehealth appointments?

Yes. Telehealth (phone or video) consultations are available. Under current Medicare rules, telehealth is generally bulk-billable only for patients who have had a face-to-face appointment with a doctor at the practice in the last 12 months. Book online or call (02) 9476 2255.

Where is the clinic located?

Suite 6, Ground Floor, 32 Florence Street, Hornsby NSW 2077. We are a short walk from Hornsby train station and have on-site and street parking nearby.

How do I register for MyMedicare with your practice?

MyMedicare is a voluntary patient registration scheme. Ask reception for a registration form at your next visit, or complete registration via your Medicare online account. Registration lets your usual GP provide better-coordinated care, including longer telehealth consultations bulk-billed under Medicare.

What are your opening hours?

Monday to Friday 8:30am – 6:00pm, and Saturday 8:30am – 12:00pm. Closed Sundays and public holidays. For after-hours care, contact the National Home Doctor Service or call 000 in an emergency.

How do I raise a concern or complaint about my care?

We welcome feedback. Please speak to our Practice Manager in the first instance, or email us. If your concern is not resolved, you may contact the NSW Health Care Complaints Commission (HCCC) or the Australian Health Practitioner Regulation Agency (AHPRA).

National Lung Cancer Screening Program Launches 1 July 2025

Florence Street Family Practice is committed to your health and well-being. Starting 1 July 2025, the Australian Government’s National Lung Cancer Screening Program will offer free low-dose CT scans to eligible individuals, aiming to detect lung cancer early when treatment is most effective.

Who is eligible?

Individuals aged 50 to 70 years
Current or former long-term smokers
Those without current symptoms of lung cancer
